Discharge and Document Retrieval Guide

Patients discharged from inpatient care are discharged from 7:00 to 18:00 on weekdays, and from 8:00 to 15:00 on Fridays and holidays.
The discharge order will be based on the treating physician's instructions, and the patient's file will be completed by the ward secretary and sent to the discharge department. Discharge will be based on the duration of hospitalization and the treatment procedures performed within a specified timeframe.
All tariffs at the center are calculated based on the Health System Evolution Plan and according to the Relative Value Book of Medical and Diagnostic Services for the current year. The invoice will be delivered to the patient accordingly.
The medical expenses of patients with supplementary insurance coverage under contract are calculated according to the agreement with insurance companies. Any expenses beyond the commitment of those organizations will be received in cash.


If your patient settles the hospital bill directly upon discharge and at the same time has Supplementary insurance They can proceed with the following steps to submit documents to the insurance and receive reimbursement:

Completing the insurance document request form upon discharge.
Contacting the Medical Records Unit at 29121088-29121087 one week after the discharge date to follow up on the documents.
If the documents are ready before one week, the Medical Records Unit will contact you.
Once you are sure that the documents are ready, you should visit the Medical Records Unit located on the first floor during non-holiday weekdays from 8 AM to 3 PM and on Fridays from 8 AM to 1 PM.
It should be noted that documents are only provided to the patient in one copy. If the patient has multiple insurances, it is necessary to make copies of the documents for each insurance. When visiting the primary insurance, the copied documents should be presented for reconciliation with the original documents for supplementary insurance purposes.
The documents will only be provided to the patient once, and duplicate copies will not be issued if they are lost.Documents will only be provided to the patient themselves or to first-degree relatives, including father, mother, spouse, siblings, and children, upon presentation of a valid identification card.If someone other than the aforementioned individuals wishes to obtain the documents, they must provide a copy of the patient's national ID card along with a consent letter from the patient for the introduction of the recipient, in addition to the recipient's national ID card, birth certificate, or identification card.If the patient is not residing in Tehran, for the convenience of the patients, the documents will be sent to the address specified on the form.
It is necessary to mention that all patients must attach their insurance documents to the invoice at the time of discharge.If the patient has visited the hospital with a referral letter, in this case, the hospital's insurance unit will send the patients' documents to the relevant insurance company. Patients can contact the hospital's insurance unit at 29121098 to inquire about the process of sending their documents.Any unauthorized access to or tampering with the contents of the medical records, for any purpose, is considered a crime and constitutes an infringement of the relevant legal provisions regarding document tampering, for which appropriate penalties are defined.
